Will be possible. Some countries have a National Health system whereby you have a number (NHS), I believe there are plans to introduce an ID card under the disguise of a Health Card in Australia that will have a chip in it with all your info, i.e., whether or not you have been vaccinated or not.
In NZ, thanks to bill gates of microsoft fame, there is a National Immunisation Register that keeps track of who has been vaccinated. So, as you can see, they do track these things
